  • Date: when the decision was made, if recorded
  • Decision makers: who decided, if recorded

Two or three sentences on the situation and the question.

  • A force that shapes the choice.

Considered options​

Decision outcome​

Chosen option, and why.

Consequences​

  • Good: what gets better.
  • Bad: what gets worse or what must now be handled.

Confirmation​

How anyone can check the decision is followed: a test, a script, a manifest.
